A written commitment regarding this must be submitted to the college registration office; otherwise, their acceptance will be canceled.<\/strong><\/p><p><strong style=\"font-size: 16px;\">2- A colored copy of the Iraqi nationality certificate and civil status identity card or unified national card.<\/strong><\/p><p><strong style=\"font-size: 16px;\">3- Three recent photographs.<\/strong><\/p><p><strong style=\"font-size: 16px;\">4- Medical examination form (according to Health Fitness System No. 5 of 1990 and its specific regulations and conditions), with attention to the following:<\/strong><\/p><p><strong>a. The student will not be registered if the mentioned form is not submitted at all.<\/strong><\/p><p><strong>b. The original form must be submitted; copies will not be accepted.<\/strong><\/p><p><strong>c. The results of the examination will be noted by the authority to which the student has applied directly. If the student is deemed unfit for the program they have applied to, the \u201cHigher Education Authority\u201d will be consulted regarding their acceptance based on their health fitness.<\/strong><\/p><p><strong>d.
